New contract: because I’m worth it!

As I sit in my practice, I can only feel that it is like being in a political dojang. The time has come to face the issue that has been swept under the carpet for the past year. It is the most radical issue that most practitioners have faced in the last
15 years.

Beating your opponents

Sun Tzu was a supreme warrior and devised a game plan to beat his opponents with strategy. It was developed over 2,000 years ago and its principles are still applied to all aspects of life today.

His game plan was: ‘Thus, those that win 100 triumphs in 100 conflicts, do not have supreme skill. Those who have supreme skill, use strategy to bend others without coming to conflict.The ideal strategy therefore is to thwart a plan. The next best is to thwart a negotiation.The next best is to thwart a strategy.’
Your local TPCT will be coming to visit all NHS dentists regarding your new contract and what UDAs they expect to be getting out of you. The TPCT knows its game plan. It has been allocated x amount of money to buy for services at local level.

What are you worth?

You’ll be expected to agree what level of service you’ll be offering to the TPCT. They’ll have their figures of what they think you are worth to them. In a previous article I discussed how as practitioners we were like mushrooms (always in the dark!) Then came the PDS contract and I thought that this is starting to let some light on NHS dentistry. However, at present all I can say is that we have been plunged into a blackout at the moment.

Commodity battle

For the practitioners who signed up for the PDS contract it appears that as the accountants look at their figures they’ll see the performers/providers slacking off! Looks like prevention still can’t be offered and has to replaced by fill/drill.
Patients are not commodities. If you work in low fluoride, social economic disadvantaged areas surely trying to maintain their oral health is very different than fluoridated water and social class one areas. It appears that what we’ll get is the UDA treadmill, with practices getting different amounts on a local level or worse a north/south divide.

High Noon

The time will come and you will have to confront the TPCT for talks about your contract. This is something you can’t bury your head in the sand and hope it will go away. Will you buckle and sign for anything they give you, leave completely and go private or do something else with your life?

Sun Tzu also said:

‘The inferior politic is to attack a fortified area. Attacking a fortified area is an art of last resort; those skilled in executing a strategy, bend the strategy of others without conflict; uproot the fortifications of others without attacking; absorb the organisations of others without prolonged operations.’
The TPCT will want to get the practitioners to sign themselves away for as cheap as possible. Practitioners willl have to defend their corner as otherwise your practice may soon fall to next to nothing

Settled dust?

After all the trauma of it all has subsided, this agenda will melt into another illusion of NHS progress, I think not. The TPCT’s will all reshuffle and amalgamate. Patients will still need to be treated, staff still to be paid and you will still need to generate an income for yourself. This time what speed have you set the UDA treadmill?

Hopefully, you can negotiate a deal with your TPCT. Remember just be like the Max Factor girls: ‘Because I’m worth it’.
